Midwest Palliative & Hospice CareCenter works to ensure that people of any age have access to palliative care if they face a serious illness or the end of life. They are committed to meeting our community’s need for compassionate, high-quality healthcare and support.

Services include hospice care, palliative care consultations, symptom management, care coordination and grief support.

The organization works with hundreds of healthcare providers in the area, including physicians, other practitioners, hospitals and senior care facilities in order to deliver care and support to patients and their families. In addition, CareCenter offers educational opportunities to community and religious groups as well as to healthcare professionals.

WHAT IS PALLIATIVE CARE? * Focuses on promoting quality of life and reliving suffering. * Interdisciplinary, comprehensive medical approach that addresses physical, psychological, spiritual and practical concerns. * Provided along with treatments that have cure and recovery as a goal. OR * Provided as the sole focus of care, as in hospice.

SERVICES Hospice Care Team-based care for patients (and their families) who are diagnosed with potentially terminal illnesses. Physician, nurse, social worker, certified nurse’s aide, chaplain and volunteer—all are part of the team that provides care, comfort and support. The hospice team focuses on easing physical and emotional pain as well as addressing fears and day-to-day concerns. This helps the patient and family achieve a better quality of life for the remaining days, weeks or months. Care is provided where the patient lives, and inpatient care is available when medically necessary. Palliative Care Services Specialists in palliative medicine are available at the request of primary care and attending physicians for consultation about the care of patients who are living with serious, complex illnesses. Through care planning and coordination, the focus is to improve quality of life and help the patient live well, whatever the diagnosis. Grief Support Offered at no cost, the organization’s bereavement services help individuals adjust to the loss of a loved one. Adults, teens and children can participate in the programming—individual counseling; a variety of groups for different types of loss; or opportunities to engage in creative expression through art, music and other workshops. Education “”Embrace Your Health”” is a health and wellness series offered free each year to the community at CareCenter; some of the topics they have featured include qigong, self-care, caring for a dementia patient and healthy eating. CareCenter representatives also welcome the opportunity to visit community or faith-based organizations. Trained professionals and volunteers can speak about a variety of topics (e.g., advance directives, the basics of hospice) in the format that best meets the group’s needs.
